Inclusive Alliance Awarded $50K Grant from Health Foundation for Western & Central New York

The Inclusive Alliance, an Independent Practice Association of health and human service nonprofit agencies serving the counties of Cayuga, Cortland, Madison, Onondaga, and Oswego and beyond, announced today that they have been awarded a grant in the amount of $50,000 from the Health Foundation for Western & Central New York. Inclusive Alliance will use the funding to further develop their infrastructure and capacity to support member-provided programs and services to the Central New York community. The Health Foundation for Western & Central New York is also supporting technical assistance from the Western New York Integrated Care Collaborative (WNYICC).

The infrastructure supported by this grant will enable local planning and development of the Pathways Community HUB model of community-based care coordination and billing support for agencies that supply the National Diabetes Prevention Program, a lifestyle change coaching program. Building member capacity to deliver these evidence-based programs meets both Inclusive Alliance’s mission of advancing the growth and quality of cost effective and inclusive individual services for children and adults through innovation, collaboration, and coordination as well as its purpose of preparing its members for value-based payment and managed care.

“This funding & technical assistance will support planning, development, and capacity building for the central infrastructure that supports our growing network of diverse organizations,” said Lauren Wetterhahn, Executive Director of Inclusive Alliance. “Through collaboration, our network partners with foundations, governmental agencies, health insurance providers, and other non-profit agencies to deliver and expand innovative, evidence-based programs that meet key community needs and to demonstrate the impact of those services on health outcomes.”
